ORO-DENTAL HEALTH: POTENTIAL TECHNICAL DIFFICULTIES IN CAST METAL FRAMEWORKS OF FIXED PARTIAL DENTURES

Introduction: Preoccupations for oro-dental health are a constant practice of the Romanian healthcare system, as oral diseases are getting higher prevalence and alter the individual’s quality of life. The potential technical difficulties found in fixed partial denture technology are a reality, being a fact of observation, not a pretext for theoretical discussions. Their occurrence is always a particularly difficult moment, being mostly noticeable by their detection rather than by the impossibility to remedy them. Material and Methods: We aimed to analyse 50 cast frameworks of fixed partial dentures with the aid of the magnifying glass. Results: In the 50 samples of fixed partial denture frameworks, there were observed the following aspects and were found the following results (in percentage). The results at the level of cast frameworks are: the joint of the dental bridge with the aggregation element - 66%, the right option and correct modelling of the dental bridge reported at edentulous ridge - 85%, the oral arch framing, respecting the vestibular and oral curvature - 89%, the contact with the contiguous teeth - 68%, occlusion performing – 84%. The main casting defects detected in analysis with the magnifying glass are: positive defects (internal or external porosity), negative defects (partial or total), volumetric defects, and distortions. They include roughness, spherical and lamellar protuberances, porosity, marginal discrepancies and volumetric casting defects. Conclusions: The errors are sometimes the result of rather isolated causes, or on the contrary of some cumulated causes, originally having the following factors: insufficient knowledge of the general concepts guiding the specialist to choose the best solution for fixed prosthetic treatment, inappropriate preparation of the prosthetic field regarding the technical requirements, followed by inaccurate impression, technological executions accompanied by unacceptable negligence and tolerability, results of insufficient knowledge of the theoretical concepts by the technician, use of low quality materials during technological stages, accepting of the patient influence. Dental alloys or frameworks are a vital part of the process of performing a treatment plan and maintaining a good overall health. This study also involves a social component, meaning an appropriate approach to oral health and dental care for patients whose treatments include partial fixed dentures.
